Formal surveys work well, and with the potential use of online techniques, it is much more efficient and effective these days. The bottom line is to ASK, either at the point-of-sale, or just after it. I make every attempt to get feedback from my clients as soon as my project is complete, and wherever they agree, I get a video testimonial from them.

The best way of getting feedback would be through
contact form, telephone calls, social media accounts, forums, live chat, surveys, reviews, e-mail follow ups and so on.

These would be the most prominent one, I suppose. Unless we're to render a valuable service, we aren't able to get with the positive suggestions.

"BEST"?

I guess there is no BEST... but lots of good ways.

First: ask. Ask how ever you can. If someone joins one of your auto responders, put a short questionnaire as the second message (WELCOME them with the FIRST). We always ask, "How did you find us?"

Ask "what attracted you to join us" after they log in.

In a restaurant, we recommend that the owner walk around to each table to see how things are going. Knowing that most people won't really complain, the owner then leaves a very short, anonymous questionnaire and asks people to fill it out. AND, the owner tells people that if they text in their name to the phone number he gives them, he/she will buy them dessert.

MOST people don't ask, so they don't know. Find a way you can ask and keep doing it.

I've always felt that having a feedback button on the site (especially if its a consumer facing startup) would decrease the credibility of the offering.

What I've tried to do is email the first 500 users directly and try and get some to email back and provide feedback. Also leveraging online networks and email mailing lists I'm on to distribute surveys, solicit feedback and start discussions works best.

Also check out a cool new service called intercom.io (too expensive for me but great service nonetheless), which is great for keeping in touch with your users without having to add a feedback/support button. Also a very new service called Sunny Trail, which digs out info on your users and lets you know who is influential or not.

Finally check out Wahooly. Fantastic service for startups looking to connect with online influencers. Can greatly help promote, interact with and engage for your startup.
